STEP 1:

Registration Packet A

Prospective students in the Colts Neck Township School District must be pre-registered in Genesis, our student information system. Step 1 is to complete Registration Packet A above. Please complete all required, and applicable, information. (If a section does not apply to you, you must check "No or N/A")

STEP 2:

Genesis Student Registration (click here)

Pre-registration does not need to be completed all at once, use these Directions for Account Creation (only if you plan to upload documents at a later date). For help logging back in to continue the registration process, use these Directions for Returning to Registration. When you Pre-register your child(ren) in Genesis, please be aware you MUST UPLOAD the completed Registration Packet A in this Step.

*It is suggested to scan documents with your cell phone if you do not have a scanner. Instructions to help with this can be found HERE.)

REQUIRED HEALTH FORMS/RECORDS

Pre-entry Physical - doctor to complete and stamp

Immunization Record - obtain from doctor

Health History - parent can complete this form
