**FOR PURPOSES OF SCHOOL ENTRY SECURITY, YOUR DRIVER'S LICENSE MUST HAVE THE SAME LAST NAME AND ADDRESS AS YOUR CHILD OR OTHER PROOF MUST BE PROVIDED**

STEP 1:

Registration Packet and Other Documents

A. Registration Packet A (Haz clic aquí para la versión en español)

B. Registration Packet B - (if applicable, this packet includes the following )

1. Certificate of Domicile - if you reside with another family in Colts Neck

2. Sworn Statement of Tenancy - to be completed if you Rent or Lease

3. Non-Parent/Student Affidavit - to be completed if you are the student's Guardian                                                                           

• A CHILD MUST BE FIVE (5) YEARS OLD ON OR BEFORE OCTOBER 1ST TO BE ELIGIBLE TO ENTER KINDERGARTEN
• MUST BE DOMICILED IN COLTS NECK WHEN SCHOOL BEGINS

Genesis Student Registration*

ALL DOCUMENTS MUST BE UPLOADED DURING THIS STEP  

Directions for Account Creation  (only if you plan to upload documents at a later date). For help logging back in to continue the registration process, use these Directions for Returning to Registration. When you Pre-register your child(ren) in Genesis, please be aware you MUST UPLOAD the completed Registration Packet A in this step.

Required Health Forms & Records

  (click on the Pre-entry Physical and Health History to obtain these forms)

Pre-entry Physical - (Must be within a year)

Health History - (parent can complete this form)

Immunization Record - (obtain record from doctor) 

*PLEASE SUBMIT ALL HEALTH RECORDS TO THE RESPECTIVE SCHOOL NURSES PRIOR TO THE START OF SCHOOL*
